2. Consent to Record and Use Media

I hereby grant Wuji Xiao Yao Kung Fu School and the Texada Wuji Xiao Yao Athletic Association (TWXYAA) permission to photograph, film, and/or audio record me (or my child, if applicable) during classes, workshops, demonstrations, community events, and related activities.

This consent includes the right to use, reproduce, publish, and distribute these recordings for purposes such as:

• Promotional materials

• Social media posts

• Website content

• Educational materials

• Posters, brochures, and community outreach

• Archival documentation of school activities

I understand that these materials may be edited, combined with other media, and used in various formats, including digital and print.

3. Rights and Compensation

I understand and agree that:

• I will not receive financial compensation for the use of these recordings.

• All media created becomes the property of the Wuji Xiao Yao Kung Fu School / TWXYAA.

• My name may be used in connection with the media unless I indicate otherwise below.

4. Opt-Out Options

I may request:

• Limited use (e.g., internal documentation only)

• No facial images

• No video

• No audio

5. Revocation of Consent

I understand that I may revoke this consent at any time by providing written notice. Revocation will not apply to media already produced or published before the notice is received.

6. Release of Liability

I release the Wuji Xiao Yao Kung Fu School, TWXYAA, its instructors, volunteers, and representatives from any claims related to the use of these recordings, including but not limited to claims of defamation, invasion of privacy, or misrepresentation.
